Jefferson Healthcare and the Activities, Health & Fitness Committees of South Bay Community Association will host an educational seminar on advanced care planning on Tuesday, Jan. 17 at the Port Ludlow Bay Club.
The presentation is at 3 p.m. and will be led by Mary Fortman.
Fortman is a licensed independent clinical social worker and will share information on advanced directives, such as planning elements and necessary documents.
Organizers of the event said the seminar is intended to prompt people to think critically about important decisions when illness and a medical crisis occur and how to handle them.
Fortman has worked in social work and discharge planning at Jefferson Healthcare for 14 years and has an in-depth understanding of the challenges patients and families face when they haven’t done advanced care planning.
