Jefferson Healthcare said Monday it has received the first shipment of Pfizer’s “baby” pediatric COVID vaccine for patients 6 months to 4 years old. Caregivers can schedule vaccine visits for children 6 months to 4 years old by calling or texing Sheridan Clinic Primary Care at 360-379-8031 or Townsend Clinic Primary Care at 360-385-5388 to schedule vaccination visits.
Children are required to be accompanied by their legal guardian or have an original Patient Acknowledgement form signed by a legal guardian. Acknowledgment forms can be downloaded from jeffersonhealthcare.org/covid-19-vaccine.
